How should I prepare for my first visit?
Please arrive at least 15 minutes prior to your appointment time to fill out our Initial Intake form and / or to discuss any insurance details if applicable. Alternatively, the forms are available online on our website under the FORMS tab and can be printed out and completed prior to coming in for your session. Please remember your appointment will begin promptly at the scheduled time.
What should I wear?
If the focus is on your upper body, please wear or bring a loose t shirt or tank. Medical gowns are also available if needed.
If the focus is on the lower body, please wear or bring loose track pants or shorts. Running shoes/ comfortable footwear is strongly recommended.
What should I bring with me?
It is your responsibility to bring any physician referrals or test results (x-ray/ultrasound/MRI) to your appointment if you have done previous investigation. It is important and beneficial that the therapist has as much information as possible pertaining to the area being assessed.
CANCELLATION POLICY - 48 HOURS NOTICE REQUIRED
We require 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, will be charged a cancellation fee of $50.00 directly billed to the credit card on file. If the appointment is cancelled or missed within 24hours, the full session charge will be applied. Third party sponsors do NOT cover this fee. (E.g. EHC, Auto insurance and WSIB)
