48 HOURS NOTICE REQUIRED - CANCELLATION POLICY
We require 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, will be charged a cancellation fee of $50.00 directly billed to the credit card on file. If the appointment is cancelled or missed within 24hours, the full session charge will be applied. Third party sponsors do NOT cover this fee. (E.g. EHC, Auto insurance and WSIB)

Pulin Shah - Registered Physiotherapist
Pulin has been a registered Physiotherapist with the College of Physiotherapy of Ontario and a good standing member with the Physiotherapist Association. His years of experience have primarily been in orthopaedic and sports injury management.
He also gained his clinical experience as a staff Physiotherapist at the University health network at Toronto General Hospital, York Central Hospital and Humber Hospital, where he primarily treated Orthopaedic and acute care patients, oncology and neurological patients.
Pulin has continued his post-graduate education in manual therapy, McKenzie mechanical diagnosis and therapy, and medically therapeutic exercise techniques, including core stability techniques.
Pulin has regularly taken courses in specific joint injury management, soft tissue release techniques, exercise prescription, neuro-proprioceptive taping, Pilates for scoliosis, and Nordic pole walking. Pulin is an avid athlete and participated in many marathons and road races. During his free times Pulin enjoys long distance running, downhill and cross country skiing, competitive sports, weight training and hiking.
Hirvi Shah - Registered Physiotherapist
Hirvi Shah is a registered Physiotherapist and is in a good standing with the College of Physiotherapy of Ontario and the Canadian Physiotherapy Association. She has been a member of our team since 2014.
Hirvi has over 9 years of experience and uses advanced techniques which include active release therapy, myofascial release, manual therapy and therapeutic taping. She treats a wide variety of clients who suffer from sports injuries, motor vehicle accidents, workplace injuries and every day aches and pain.
Hirvi also has a special interest in pediatric physiotherapy. She has worked with children as young as 20 days and treated conditions such as Cerebral palsy, delayed milestones, autism, Hypotonic babies, torticollis, plagiocephaly and prematurity. She has extensive training in bobath technique, neuro development technique and sensory integration.
She provides home/community based Rehab for high risk infants by providing education and training to parents.
Hirvi is committed to continuing her professional development by attending courses to ensure that the physiotherapy treatment provided follows evidence based practise and that the most up to date techniques are used.
In her spare time Hirvi enjoys swimming, biking and travelling.
Shruti Mehta- Registered Physiotherapist
Shruti is a registered Physiotherapist with the College of Physiotherapy of Ontario. She believes in patient centred care and believes to work in partnership with each individual to facilitate their healing process and improve their quality of life. Her professional goal is to build a practice based on relationships with her patients and to service the community.
Shruti is trained in McKenzie Method of Diagnosis and Treatment (Part A), Mulligan concept of Manual therapy, Prenatal and Postnatal Physical Therapy.
Her areas of interest include Musculoskeletal and Sports injuries , Pain Management , post- operative rehabilitation, back injury prevention and rehabilitation. She firmly believes in treating the root cause of the issue based on research and evidence rather than working just symptomatically. Her mode of practice is to provide care, education as well as access to all her patients whenever needed.
When not treating clients, she keeps herself busy with her twin daughters, listening to music, travelling and loves cooking!
Matthew Chan - Registered Physiotherapist
Matthew completed his Masters degree of Physiotherapy at McMaster University after completing his Bachelor’s in Kinesiology at The University of Waterloo. He has also completed his Advanced Integrated Musculoskeletal Program Level 1.
Matthew is Canadian born, and he has also lived in Hong Kong and Taiwan. He is fluent in both Cantonese and Mandarin.
Matthew has a passion for helping people achieve their optimal health and personal fitness goals. He strives to deliver high quality care by incorporating evidence-based research and a holistic, client-centered approach in his practice. He places strong emphasis on proper education and utilizes a combination of treatment techniques, such as therapeutic exercises, modalities, mobilization and soft tissue therapy to treat and prevent future injuries. He has experience working in both the hospital and private practice environments, which has allowed him to work with clients with various conditions, including sport and motor vehicle injuries, chronic pain, stroke and post-cardiac rehab.
Prior to Matthew becoming a physiotherapist, He was a professional swimmer who competed in national and international levels. He is also a certified personal training specialist, who enjoys physical challenges. Aside from his practice, Matthew enjoys hiking, traveling and trying new foods. He also takes pride in being a lifelong learner and is interested in pursuing certifications in vestibular rehab and concussion management as well as manual therapy and acupuncture.
Tomo Monsma - Physiotherapy Assistant
Tomo joined Active Life in 2017. She is a foreign trained Physiotherapist from Japan. She works as a Physiotherapy Assistant here in Canada and has extensive experience in patient care. Her mission is to make client's experience at Active Life as comfortable as possible. Tomo enjoys playing baseball, practicing her guitar, and spending time with her family in her spare time.
Pegah Hajizargarbashi - Registered Chiropractor
Dr. Pegah is a holistic, evidence-based practitioner who has obtained her Doctor of Chiropractic degree from the Canadian Memorial Chiropractic College (CMCC).
Dr. Pegah has always been passionate about promoting a healthy and active lifestyle and helping others to achieve their maximum potential in health. She is experienced in treating both acute and chronic neuromusculoskeletal conditions. To achieve the optimal functioning and well-being for patients , she uses diversified manual technique, patient education, soft tissue techniques, and exercise prescription.
Tharshni Velauthapillai - Registered Chiropractor
Dr. Tharshni is Toronto-born and raised, and has always had an interest in health and wellness. She graduated as a Doctor of Chiropractic (DC) from the Canadian Memorial Chiropractic College in 2019. She practiced as a chiropractor for 2 years in Calgary, and began practicing in Toronto in 2021.
Dr. Tharshni uses a variety of techniques for treatment including spinal and extremity manipulative therapy (adjustments), mobilizations, soft tissue therapy, instrument-assisted therapy, rehabilitation and exercise prescription. She has extensive experience treating car accident injuries, workplace injuries, chronic pain, pregnancy-related pain, and athletic injuries.She tailors each plan of treatment to the needs of her patients, and has experience treating a variety of musculoskeletal conditions. Her goal is to get her patients to be pain-free, active, and strong. She uses both preventative care and maintenance care to ensure her patients have long-lasting relief.
Phyllis Lin - Registered Massage Therapist
Phyllis Lin has been a part of the Active Life Clinic since 2011. She is fluent in English, Mandarin, and Cantonese. She graduated with a diploma in Massage and Esthetics in 1997 and continued the education in Wellspring College from 2009-2010 and became a Registered Massage Therapist of Ontario (CMTO) in 2010. Phyllis has experience in Swedish massage, advanced massage (Trigger point releasing, friction, joint play, myofascial release, etc.), Thai massage and hot stone massage techniques. By using a variety of techniques she provides stress and pain reduction, injury prevention, deep tissue and relaxation massage. Phyllis continues to attend the education workshop to provide more professional and high quality services.
Judy Chen - Registered Massage Therapist/Registered Acupuncturist
Judy is a talented member of our team who is fluent in Mandarin, Cantonese and English and loves engaging with her patients. She is a graduate of the Wellspring College and received her qualifications as a registered massage therapist in 2010. Judy graduated from Heilongjiang University in 1996 as a Doctor of Traditional Chinese Medicine and Judy has had over twelve years of hands-on experience in treating patients with chronic pain through Tuina (Chinese Massage) and acupuncture.
She also specializes in employing Swedish massage and advanced massage techniques (e.g. trigger point release, joint play and remedial exercise) to treat muscular pain, headaches, joint dysfunction and sports related injuries. She especially enjoys working with pre- and post- partum women as well as young children. Judy is a bright and energetic individual who firmly believes in providing her patients with the world-class health care seen at Activelife.
Mable Li - Registered Massage Therapist
Mary Jeane Paronable - Registered Massage Therapist
Mary Jeane believes that her professional background of how she graduated with a Bachelor’s Degree in Physiotherapy from the Philippines has helped her become a well rounded Registered Massage Therapist. She has studied several courses to enhance her skills in General Swedish Massage, Deep Tissue Massage, Myofascial Cupping, Post Surgical, Treatments, Thai Table Massage, Craniosacral Techniques, and Indian/Ayurvedic head, Massage. She incorporates these techniques to alleviate pain, muscle tension, headache/migraines, trigger points, improve joint stiffness and decrease stress. In her downtime, Mary enjoys spending time with her family and friends.
Ada Chiu - Registered Chiropodist
Ada Chiu D.Ch. is a dedicated Chiropodist with experience in treating and preventing diseases and dysfunctions of the foot. Ada graduated from the University of Toronto with a Bachelors of Science degree in Human Biology. She then received her Chiropody training at the Michener Institute for Applied Health Sciences in Toronto, where she underwent 3 years of practical training in podiatric medicine, biomechanics, orthotic therapy, anesthesia, nail surgery, physical therapy and podopediatrics.
Ada is a registered member with the College of Chiropodists of Ontario. As a primary foot care provider, Ada believes that a Chiropodist or Podiatrist can help children, adults and seniors with a variety of foot problems. In children, early detection of many foot problems such as flat feet, in or out toeing can prevent more serious complications from developing later in life such as knee and lower back pain. The geriatric population can also benefit from Chiropody or Podiatry services for treatment of painful lesions resulting from the effects of diabetes, rheumatoid or osteoarthritis and biochemical anomalies.
Ada is excited to be part of the Active Life Physiotherapy team. She is fluent in Cantonese and looking forward to bringing the best of foot care to you and your family.
